The procedure involves real-time imaging and must include documentation of fetal size by measuring standard growth parameters and assessing amniotic fluid volume. The standard biometric parameters typically measured include the Biparietal Diameter (BPD), Head Circumference (HC), Abdominal Circumference (AC), and Femur Length (FL). These measurements are then used to calculate an Estimated Fetal Weight (EFW) and determine the fetal growth percentile relative to gestational age. In addition to growth, the physician or sonographer re-evaluates specific organ systems that were flagged as abnormal, suspicious, or poorly visualized on a prior scan. Procedure Steps
- Verify the clinical indication and review findings from the most recent prior obstetric ultrasound.
- Apply acoustic coupling gel to the patient's lower abdomen.
- Use a transabdominal transducer to identify fetal position, number, and presence of cardiac activity.
- Measure the Biparietal Diameter (BPD) and Head Circumference (HC) at the correct anatomical plane (level of thalami and cavum septum pellucidum).
- Measure the Abdominal Circumference (AC) at the level of the junction of the umbilical vein and left portal vein.
- Measure the Femur Length (FL) of the long axis of the bone, excluding the epiphysis.
- Perform a targeted re-evaluation of the specific organ system(s) identified as abnormal or suspicious in previous reports.
- Assess amniotic fluid volume using the four-quadrant AFI method or Single Deepest Pocket measurement.
- Calculate the Estimated Fetal Weight (EFW) and compare it with prior measurements to determine growth velocity.
- Document findings with permanent images and generate a formal written report.
Coding Guidelines
- CPT 76816 should not be reported for the same fetus on the same day as 76801-76812 or 76815.
- This code is 'per gestation.' For multifetal pregnancies (e.g., twins), report 76816 for the first fetus and 76816 with modifier 59 (or XS) for the second fetus, or follow specific payer instructions for twins.
- This is a follow-up exam; a comprehensive initial scan (76805 or 76811) should have been performed previously.
- If only a quick check of fetal position or heart rate is performed without biometric measurements, use 76815 (Limited) instead of 76816.
- Do not report 76816 if the purpose of the exam is purely for biophysical profile (BPP); use 76818 or 76819 for BPP.
- The report must include comparisons to previous measurements to justify the follow-up status.
