12001
Simple Repair of Superficial Wounds (2.5 cm or less)
CPT code 12001 is designated for the simple repair of superficial wounds that measure 2.5 cm or less in length. This procedural code applies specifically to wounds located on the following anatomical sites: the scalp, neck, axillae, external genitalia, trunk, and/or extremities, including the hands and feet. According to the CPT manual, a simple repair is defined as a closure used when the wound is superficial, involving primarily the epidermis or dermis, or even subcutaneous tissues without significant involvement of deeper structures. Such a repair requires only a single layer of suturing, stapling, or the application of tissue adhesives like Dermabond. The procedure encompasses several clinical steps, beginning with a thorough evaluation of the injury to ensure it does not involve deeper layers such as fascia or muscle, which would necessitate an intermediate or complex repair code. The service includes the administration of local anesthesia, such as a lidocaine injection or topical anesthetic, followed by the meticulous cleaning and irrigation of the wound to remove foreign matter and reduce infection risk. While simple exploration is included, extensive debridement of heavily contaminated wounds may shift the coding to a higher complexity level. Providers must document the exact measurement of the wound and its specific anatomical location to justify the selection of 12001. If multiple wounds of the same complexity and anatomical grouping are present, their lengths must be added together rather than reported individually. It is also important to note that using only adhesive strips (e.g., Steri-Strips) to close a wound does not satisfy the requirements for 12001 and is instead considered part of an evaluation and management service.
Clinical Indications
- Superficial laceration of the scalp, neck, trunk, or extremities
- Wound depth limited to the epidermis, dermis, or subcutaneous tissue
- Wound length of 2.5 cm or less
- Primary intention healing achievable with single-layer closure
- Traumatic cuts requiring sutures, staples, or tissue adhesive
Procedure Steps
- Perform clinical assessment of the wound to determine depth and extent of injury.
- Administer local anesthesia via injection or topical application.
- Irrigate the wound with sterile saline to remove contaminants and debris.
- Achieve hemostasis through pressure or minor cauterization.
- Approximate the wound edges using a single layer of sutures, staples, or tissue adhesive.
- Clean the surrounding skin and apply a sterile dressing or antibiotic ointment.
- Document the wound length, location, and closure technique in the medical record.
- Provide patient instructions for wound care and follow-up.
Coding Guidelines
- Wound length must be measured and recorded in centimeters.
- Combine lengths of multiple wounds within the same anatomical group (e.g., two 1 cm wounds on the arm = 2 cm, coded as 12001).
- Do not combine wounds from different anatomical groups (e.g., a scalp wound and a face wound).
- Simple repair includes local anesthesia and simple exploration.
- Adhesive strips alone (e.g., Steri-Strips) are not coded as a repair; they are included in the E/M service.
- If debridement is extensive or requires significant additional work, refer to debridement codes or intermediate repair codes.
- 12001 is for wounds 2.5 cm or less; for larger simple wounds in these areas, use 12002-12007.
