Clinical Indications
- Diagnosis of acute monoarticular arthritis (e.g., septic arthritis, gout, pseudogout) through synovial fluid analysis.

- Management of inflammatory arthropathies (e.g., rheumatoid arthritis, osteoarthritis with inflammatory flares, psoriatic arthritis) unresponsive to conservative measures.

Procedure Steps
- Patient identification, review of medical history, and obtaining informed consent.
- Careful positioning of the patient to expose the anatomical landmark for the major joint or bursa.
- Sterile preparation of the skin overlying the injection/aspiration site using an appropriate antiseptic solution.
- Administration of local anesthetic (e.g., lidocaine) to the skin and subcutaneous tissues at the entry site.
- Insertion of an appropriate-sized needle into the joint space or bursa, often guided by palpation or, if necessary, by imaging (e.g., ultrasound, fluoroscopy).

Coding Guidelines
- CPT code 20610 includes both aspiration and/or injection. If both procedures are performed during the same encounter at the same major joint/bursa, only 20610 should be reported once.
- When multiple major joints or bursae are injected/aspirated during the same encounter, 20610 may be reported for each distinct site. Modifier 59 (Distinct Procedural Service) or an appropriate anatomical modifier (e.g., RT, LT) should be appended for each additional site to indicate multiple distinct services.
- Imaging guidance (e.g., ultrasound 76942, fluoroscopy 77002, CT 77012, MRI 77021) for needle placement is reported separately when performed and medically necessary. Documentation must clearly support the use of guidance and include a separate interpretation report.
- Local anesthesia is considered an inherent component of the procedure and is not separately billable.
