Procedure Steps
- Obtain percutaneous vascular access using the Seldinger technique, typically in the common femoral or brachial artery.
- Perform a diagnostic angiogram to identify the location, extent, and nature of the thrombus.
- Advance a guidewire across the thrombotic occlusion under fluoroscopic guidance.
- Select and prepare the mechanical thrombectomy device (e.g., AngioJet, Penumbra, or Rotarex).
- Advance the thrombectomy catheter over the guidewire to the site of the occlusion.
- Activate the device to macerate or aspirate the thrombus; multiple passes may be required to clear the vessel.
- Perform follow-up angiography to assess the degree of recanalization and distal runoff.
- Address any underlying stenotic lesions (e.g., with PTA or stenting) if necessary after the thrombus is cleared.
- Remove the device and sheath, and achieve hemostasis at the access site using manual pressure or a closure device.

- Fluoroscopic guidance and intraprocedural injections for imaging the treated vessel are included in 37184 and should not be reported separately.
