Procedure Steps
- Obtain informed consent and position the patient in a supine position.
- Administer local anesthesia and establish percutaneous arterial access, typically via the common femoral artery using the Seldinger technique.
- Advance a sheath and perform diagnostic angiography to visualize the location and severity of the iliac lesion.
- Navigate a guidewire across the stenotic or occluded segment of the target iliac vessel.
- Perform pre-dilation balloon angioplasty if the lesion is highly calcified or tight (optional but included).
- Advance and deploy a self-expanding or balloon-expandable stent across the lesion under fluoroscopic guidance.
- Perform post-dilation angioplasty within the stent to optimize expansion and ensure adequate wall contact.
- Perform a completion angiogram to assess distal flow, vessel patency, and the absence of complications like dissection.
- Remove the delivery system and sheath, and achieve hemostasis at the access site using manual compression or a closure device.
