For multiple distinct lesions or anatomical sites requiring separate CT guidance sessions, 77014 may be reported multiple times with appropriate modifiers (e.g., 59, XU) if clinically justified and documented.
- Modifier 26 should be appended if only the professional component (radiologist's work) is being billed, or the TC modifier for the technical component.
- Do not report 77014 in conjunction with other imaging guidance codes (e.g., ultrasound (76942), fluoroscopy (77002), magnetic resonance (77021)) for the same needle placement procedure, as only one form of guidance should be chosen based on the primary imaging modality used.
