80048
Basic Metabolic Panel (BMP)
The Basic Metabolic Panel (BMP), designated by CPT code 80048, is a standard group of eight individual blood tests that provide critical information about a patient's metabolism. This panel is foundational in clinical medicine for evaluating kidney function, blood glucose levels, and the balance of electrolytes and fluids within the body. Specifically, the 80048 panel includes automated measurements of Calcium (total), Carbon dioxide (bicarbonate), Chloride, Creatinine, Glucose, Potassium, Sodium, and Urea nitrogen (BUN). Each component serves a distinct diagnostic purpose: electrolytes (sodium, potassium, chloride, and CO2) are essential for maintaining acid-base balance and proper nerve and muscle function; glucose levels screen for diabetes or hypoglycemia; and BUN and creatinine levels are primary indicators of renal filtration efficiency. Clinically, the BMP is used for a wide range of purposes including routine health screenings, monitoring chronic conditions like hypertension or heart failure, and evaluating patients with acute symptoms such as weakness, confusion, or edema. The procedure involves a simple venipuncture to collect a blood sample, typically in a serum separator tube (SST) or a heparinized plasma tube. Once in the laboratory, the specimen is centrifuged, and the serum or plasma is analyzed using automated chemistry platforms. Because the components are interrelated, assessing them as a single panel provides a more comprehensive picture of a patient's internal homeostasis than any single test alone. Healthcare providers use these results to guide fluid resuscitation, adjust medication dosages (especially diuretics and blood pressure medications), and identify metabolic emergencies such as ketoacidosis or severe renal failure.
Clinical Indications
- Routine physical examination and health screening
- Monitoring patients with hypertension or heart failure
- Evaluation of renal function in patients with known kidney disease
- Assessment of electrolyte balance in patients taking diuretics or ACE inhibitors
- Screening for or monitoring of diabetes mellitus
- Pre-operative medical clearance
- Evaluation of symptoms such as fatigue, weakness, or mental status changes
- Monitoring of patients with fluid volume disorders or dehydration
- Assessment of acid-base balance
- Management of patients in acute care or emergency settings
Procedure Steps
- Verify patient identification using at least two identifiers.
- Perform venipuncture using aseptic technique to collect a venous blood sample.
- Collect the sample into a gold-top (SST) or light-green-top (lithium heparin) tube.
- Invert the tube gently 5 to 8 times to mix the additive with the blood.
- Allow the SST sample to clot for approximately 30 minutes in an upright position (if using serum).
- Centrifuge the specimen according to laboratory protocol to separate cells from serum or plasma.
- Process the specimen on an automated clinical chemistry analyzer.
- Perform quality control checks and verify the accuracy of the automated results.
- Report the quantitative results for all eight components to the ordering provider.
Coding Guidelines
- The 80048 panel must include all eight specific tests: Calcium, CO2, Chloride, Creatinine, Glucose, Potassium, Sodium, and BUN.
- If any component of the panel is not performed, the individual CPT codes for the tests performed must be reported instead of 80048.
- Do not report 80048 in conjunction with 80053 (Comprehensive Metabolic Panel), as 80053 includes all components of 80048.
- If any of the tests in the panel are performed multiple times on the same day, the panel code is reported for the initial testing, and individual codes with modifier 91 may be used for repeat tests if clinically justified.
- When 80048 is performed as part of a larger panel (e.g., 80050), report only the comprehensive panel code.
- The panel code cannot be reported with individual test codes from the same panel on the same date of service for the same patient.
