90471
Immunization Administration (Intramuscular, Subcutaneous, Intradermal, or Percutaneous); Initial Vaccine
Code 90471 represents the professional service of administering a single or initial vaccine or toxoid to a patient via percutaneous, intradermal, subcutaneous, or intramuscular routes. This procedure is fundamental to preventive medicine and public health. The administration process begins with a comprehensive review of the patient's medical history and immunization record to determine the necessity of the vaccine and to ensure it aligns with current CDC and ACIP schedules. The healthcare provider must verify the physician's order and confirm the patient's identity using at least two identifiers. A critical component of this service is the provision of the Vaccine Information Statement (VIS), as mandated by the National Childhood Vaccine Injury Act. The VIS explains the benefits, risks, and potential side effects of the specific vaccine being administered to the patient or guardian. Clinical assessment involves screening for contraindications such as previous anaphylactic reactions to vaccine components (e.g., neomycin, gelatin, or egg protein) or acute moderate-to-severe illness that might warrant a delay. The technical execution of 90471 includes selecting the appropriate route—most commonly intramuscular (IM) into the deltoid muscle for adults or the vastus lateralis for infants, or subcutaneous (SubQ) injection. The provider performs hand hygiene, prepares the injection site with an antiseptic, and uses a sterile, single-use needle and syringe. After administration, the needle is immediately discarded in a biohazard sharps container to prevent needle-stick injuries. The provider must then monitor the patient for any immediate adverse reactions, such as vasovagal syncope or allergic response. Comprehensive documentation is required, including the date of administration, the vaccine manufacturer, lot number, expiration date, the site and route of injection, and the version date of the VIS provided. This code is used for the first vaccine delivered via injection during a single clinical encounter, regardless of whether the vaccine contains one or multiple components.
Clinical Indications
- Routine childhood immunization schedules
- Adult preventive health boosters
- Seasonal influenza prophylaxis
- Travel-related vaccinations and prophylaxis
- Wound management requiring tetanus toxoid administration
- Response to infectious disease outbreaks
- Occupational health requirements for high-risk workers
- Catch-up immunization for unvaccinated or under-vaccinated individuals
Procedure Steps
- Confirm patient identity using two identifiers and verify the physician order.
- Review patient immunization history and assess for allergies or contraindications.
- Provide the relevant Vaccine Information Statement (VIS) and answer patient questions.
- Perform hand hygiene and don personal protective equipment as necessary.
- Select the appropriate vaccine and verify the lot number and expiration date.
- Aseptically prepare the vaccine and identify the anatomical injection site.
- Cleanse the skin with an alcohol swab and stabilize the site.
- Inject the vaccine using the specified route (IM, SubQ, ID) and withdraw the needle.
- Apply pressure or a bandage to the site and dispose of sharps in a biohazard container.
- Observe the patient for 15 minutes for any immediate adverse reactions.
- Document the vaccine details, site, route, and VIS date in the medical record.
Coding Guidelines
- Use 90471 for the first or only vaccine administered by injection during an encounter.
- Report add-on code 90472 for each additional vaccine given by injection at the same visit.
- Do not use 90471 if the provider performs face-to-face vaccine counseling for patients aged 18 and under; use 90460 instead.
- The vaccine product itself must be coded separately using the appropriate CPT code (90476-90749).
- If a significant, separately identifiable Evaluation and Management (E/M) service is performed, append modifier 25 to the E/M code.
- Do not report 90471 in conjunction with 90473 (oral/nasal administration) as the primary administration code if both are given; use 90471 as the primary code and 90474 for the oral/nasal component.
- Immunization administration codes include the work of the injection and the mandatory VIS provision.
