The use of this code is appropriate when at least one of four specific complicating factors is present: 1) the need to manage maladaptive communication among participants that complicates delivery of care; 2) caregiver emotions or behavior that interfere with the caregiver's ability to assist in the implementation of the treatment plan; 3) evidence or disclosure of a sentinel event and mandated reporting to a third party (e.g., abuse or neglect) with initiation of discussion of the event and/or report with the patient and other visit participants; or 4) use of play therapy, physical devices, or interpreters to overcome barriers to diagnostic or therapeutic interaction when the patient has not yet developed or has lost either the expressive language communication skills to explain symptoms and respond to treatment, or the receptive communication skills to understand the clinician if they were to use ordinary adult language. Clinicians must document the specific factors that contributed to the interactive complexity and how those factors impacted the session. It is commonly utilized in pediatric psychiatry, forensic psychiatry, or when treating patients with significant cognitive or communication impairments.

- Identify and document the primary psychiatric service performed (e.g., 90791, 90832, 90834, 90837)
- Observe and determine the presence of one or more of the four specific complexity factors defined by CPT guidelines
- Engage in specialized communication techniques to manage maladaptive behaviors or emotional barriers presented by participants
- Utilize play therapy equipment, physical devices, or external interpreters if the patient has expressive or receptive language barriers
- Address and manage high-intensity emotional reactions from caregivers or family members present during the encounter
- Perform required reporting actions if a sentinel event is disclosed and discuss the implications with the patient and involved parties
- Conclude the primary service while maintaining the complexity-adjusted communication strategies throughout the duration of the visit
- Document the specific complexity factors and the clinical rationale for reporting 90785 in the medical record
Coding Guidelines
- 90785 is an add-on code and must always be reported in conjunction with a primary psychiatric service code
- Appropriate primary codes include Psychiatric Diagnostic Evaluation (90791, 90792), Psychotherapy (90832, 90833, 90834, 90836, 90837, 90838), or Group Psychotherapy (90853)
- It cannot be reported with E/M services unless a psychotherapy add-on code (e.g., 90833, 90836, 90838) is also reported
- The code should be reported only once per session, regardless of the number of complexity factors present
- Interactive complexity is not to be reported for simple translation services or the routine presence of a parent in a pediatric session without the defined complexity factors
- Documentation must clearly state which of the four CPT-defined factors were met and how they complicated the service
- Do not report 90785 in conjunction with psychotherapy for crisis codes (90839, 90840)
