Pharmacologic management, including prescription and review of medication, when performed in conjunction with interactive psychotherapy

CPT code 90863 represents pharmacologic management services, which include activities such as prescribing new medications, adjusting existing medication dosages, reviewing medication efficacy and side effects, and providing patient education regarding psychotropic medications. This service is reported when it is performed by a physician or other qualified healthcare professional in conjunction with an interactive psychotherapy session. It specifically captures the distinct work of medication management that occurs during a psychotherapy encounter, distinguishing it from the psychotherapy itself.

Clinical Indications

  • Patients receiving ongoing psychotherapy who also require psychiatric medication management (e.g., for depression, anxiety, bipolar disorder, schizophrenia, PTSD).
  • Evaluation and adjustment of psychotropic medications due to lack of efficacy, adverse side effects, or changes in clinical status during a psychotherapy session.
  • Initial prescription of psychotropic medication in a patient concurrently starting or continuing psychotherapy.
  • Monitoring of medication adherence, response, and side effects in conjunction with therapeutic discussions.
  • Patients with complex mental health conditions benefiting from integrated psychotherapeutic and pharmacologic approaches.

Procedure Steps

  1. Assessment of the patient's current psychiatric symptoms and mental status.
  2. Review of the patient's current medication regimen, including dosages, administration, and adherence.
  3. Discussion of medication efficacy in managing target symptoms and improvement or worsening of the condition.
  4. Identification and assessment of any experienced side effects or adverse drug reactions.
  5. Decision-making regarding medication changes (e.g., initiation of new medication, dosage adjustment, discontinuation).
  6. Prescription or renewal of psychotropic medications.
  7. Education of the patient (and/or family, if appropriate) regarding medication purpose, expected benefits, potential side effects, and administration instructions.
  8. Coordination of care with other providers, if necessary.
  9. Documentation of the pharmacologic management discussion, decisions, prescription, and patient response.

Coding Guidelines

  • CPT code 90863 is an add-on code and must be reported in conjunction with an interactive psychotherapy CPT code (e.g., 90832, 90834, 90837, 90847). It cannot be billed as a standalone service.
  • This code specifically describes pharmacologic management when performed as part of or linked to an interactive psychotherapy session. If medication management occurs entirely separate from a psychotherapy session, a different Evaluation and Management (E/M) code (99202-99215) would typically be appropriate.
  • The documentation must clearly support both the psychotherapy services and the distinct pharmacologic management provided during the same encounter.
  • Do not report 90863 if the pharmacologic management component is already inherent in a separately billed E/M service and is not performed in conjunction with interactive psychotherapy.
  • Modifier 25 is generally not required for add-on codes like 90863 when billed with a primary procedure, as add-on codes by definition describe a service performed in addition to the primary service.
