Programming device evaluation, dual lead pacemaker system
CPT code 93280 describes an in-person programming device evaluation of a dual-lead permanent pacemaker system. This procedure involves the use of an external programmer to communicate with the implanted pulse generator and its two associated leads, typically one located in the right atrium and one in the right ventricle. The evaluation is a comprehensive assessment that includes the interrogation of the device to retrieve stored data, followed by iterative testing and adjustment of the device's settings to optimize clinical performance. During the session, the clinician evaluates multiple technical parameters including lead impedance to ensure physical integrity, sensing thresholds to verify the device's ability to detect intrinsic cardiac activity, and pacing thresholds to determine the minimum electrical output required to consistently capture the myocardium. The clinician analyzes histograms, counters, and stored electrograms to identify any episodes of arrhythmia, such as atrial fibrillation or ventricular tachycardia. Based on this data, the physician or qualified healthcare professional performs 'iterative adjustments'—tuning parameters such as the lower rate limit, atrioventricular (AV) delay, paced and sensed refractory periods, and rate-responsive sensors—to improve the patient's hemodynamics and symptoms. The service is not merely a data download; it requires professional analysis, a review of the patient's clinical status in relation to the device data, and the creation of a formal report detailing the findings and the specific changes made to the programmed parameters. This code is specific to dual-chamber systems and is performed in a face-to-face setting.
Clinical Indications
- Follow-up for patients with a dual-chamber pacemaker to ensure optimal device function.
- Evaluation of new-onset symptoms such as syncope, pre-syncope, or palpitations in a pacemaker patient.
- Management of suspected lead malfunction or insulation failure suggested by previous remote monitoring.
- Optimization of device parameters following changes in the patient's clinical status or medication regimen.
- Routine surveillance of battery longevity and elective replacement indicators (ERI).
- Post-procedural programming following initial implantation or lead revision.
- Adjustment of rate-responsive settings for patients experiencing exertional dyspnea.
Procedure Steps
- Position the programmer wand or utilize wireless telemetry over the patient's implanted generator.
- Interrogate the device to retrieve current settings, battery status, and stored diagnostic data.
- Perform lead impedance testing for both the atrial and ventricular leads to assess circuit integrity.
- Conduct sensing threshold tests to ensure the device accurately identifies intrinsic P-waves and R-waves.
- Determine pacing thresholds (capture) by gradually decreasing output until capture is lost, then setting a safety margin.
- Review stored electrograms and arrhythmia logs for any significant cardiac events since the last evaluation.
- Perform iterative adjustments to programmed parameters such as AV delay, output voltage, and pulse width.
- Verify the final programmed settings and ensure the device is communicating correctly.
- Generate a comprehensive report including the pre-programming data, testing results, adjustments made, and clinical interpretation.
Coding Guidelines
- Code 93280 is specifically for dual-lead pacemaker systems. For single-lead systems, use 93279. For multiple-lead (3 or more) systems, use 93281.
- This is an in-person service. If the evaluation is performed remotely, use the 93294-93296 series.
- Do not report 93280 in conjunction with 93288 (interrogation evaluation only) for the same device during the same session.
- Programming evaluations (93279-93281) include all the components of an interrogation evaluation (93288).
- If a programming evaluation is performed on the same day as an E/M service by the same physician, the E/M may be reported with modifier 25 if it is a significant, separately identifiable service.
- This code should not be reported more than once per day, regardless of the number of adjustments made.
