96372

Therapeutic, prophylactic, or diagnostic injection (specify substance or drug); subcutaneous or intramuscular

CPT code 96372 describes the administration of a substance or drug via a subcutaneous (SC) or intramuscular (IM) injection route. This service encompasses therapeutic, prophylactic, or diagnostic purposes. It includes the preparation and injection of the medication by a healthcare professional, but it does not include the cost of the drug itself. This code is distinct from intravenous injections/infusions, immunizations, or injections integral to other procedures (e.g., local anesthesia for a minor surgical procedure).

Clinical Indications

  • Administration of various medications for therapeutic effect (e.g., antibiotics, pain relievers, antiemetics, hormones, vitamin B12)
  • Administration of substances for prophylactic purposes (e.g., allergy desensitization injections, certain prophylactic medications)
  • Administration of substances for diagnostic purposes (e.g., purified protein derivative (PPD) skin test for tuberculosis screening, allergy skin testing)
  • Injection of insulin or other injectable diabetes medications by a healthcare provider (not self-administered)
  • Administration of long-acting injectable antipsychotics or other mental health medications

Procedure Steps

  1. Verify physician orders, patient identity, and medication (drug, dose, route, time).
  2. Prepare the medication (e.g., draw up the correct dose from a vial or ampule, ensure proper concentration).
  3. Select an appropriate injection site (e.g., deltoid, vastus lateralis, ventrogluteal, subcutaneous tissue of abdomen or upper arm).
  4. Perform aseptic skin preparation of the chosen injection site.
  5. Administer the injection using appropriate needle size and technique for the chosen route (subcutaneous or intramuscular).
  6. Ensure proper needle disposal and provide post-injection care (e.g., apply pressure, observe for adverse reactions).
  7. Document the medication administered, dose, route, site, date, time, and patient's reaction/response in the medical record.

Coding Guidelines

  • Report CPT 96372 for each distinct subcutaneous or intramuscular injection administered.
  • Do not report 96372 for injections that are an inherent part of another separately reported procedure (e.g., local anesthetic for a minor surgery).
  • This code is for the administration service only; the substance or drug itself should be coded separately using appropriate J codes or NDC if applicable.
  • Do not report 96372 for intravenous injections or infusions; use codes from the 96360-96368 series for these services.
  • Do not report 96372 for routine immunizations; use codes from the 90460-90474 series for immunization administration.
  • When significant, separately identifiable evaluation and management (E/M) services are performed on the same date, modifier 25 should be appended to the E/M code.
  • If multiple distinct therapeutic, prophylactic, or diagnostic injections are administered during the same encounter, 96372 may be reported multiple times, and modifier 59 (Distinct Procedural Service) may be necessary to indicate a separate and distinct service, especially if different substances are injected at different sites.
