Manual Therapy Techniques (eg, mobilization/manipulation, manual lymphatic drainage, manual traction), 1 or more regions, each 15 minutes
CPT 97140 refers to a clinical procedure where a healthcare provider, such as a physical therapist, occupational therapist, or chiropractor, performs manual therapy techniques on one or more regions of the patient's body. These techniques are characterized by direct, one-on-one contact between the provider and the patient and are delivered in 15-minute increments according to the Medicare 8-minute rule. The scope of this code encompasses various skilled manual interventions including, but not limited to, joint mobilization and manipulation, manual traction, manual lymphatic drainage, and myofascial release. Unlike simple massage therapy (97124), manual therapy is highly specific and goal-oriented, designed to increase active or passive range of motion, modulate pain, reduce soft tissue swelling or inflammation, and improve tissue extensibility. Mobilization involves passive movement of a joint within or at the physiological limit of range of motion, whereas manipulation often involves a high-velocity, low-amplitude thrust. Manual traction uses the provider's hands to apply a distracting force to the spine or extremities to relieve pressure on neural structures. Manual lymphatic drainage is a specialized technique aimed at moving interstitial fluid into the lymphatic system. This procedure is considered a 'skilled service,' meaning the clinical expertise of a licensed provider is necessary to ensure safety and therapeutic efficacy. Proper documentation for this code requires detailing the specific technique used, the specific anatomical site treated, the duration of the treatment, and the patient's immediate response to the intervention.
Clinical Indications
- Restricted joint range of motion or joint stiffness
- Musculoskeletal pain (acute or chronic) resulting from dysfunction
- Myofascial trigger points or soft tissue adhesions
- Lymphedema or significant soft tissue edema
- Post-surgical scarring or tissue tightness affecting function
- Spinal or peripheral joint subluxations
- Neuromuscular tension or hypertonicity
- Cervicogenic headaches
Procedure Steps
- Perform a comprehensive musculoskeletal assessment to identify specific areas of restriction or dysfunction.
- Position the patient appropriately to ensure comfort and optimal access to the target anatomical region.
- Explain the procedure to the patient and obtain informed consent, especially for high-velocity techniques.
- Apply specific manual force (pressure, traction, or movement) to the joint or soft tissue based on the clinical objective.
- Maintain the force or rhythmic oscillation for the duration necessary to achieve the desired physiological response.
- Continuously monitor the patient's pain levels and physical response throughout the manual application.
- Re-evaluate the joint mobility or tissue quality post-intervention to assess progress.
- Document the specific techniques applied, the region treated, and the total time spent in direct contact.
Coding Guidelines
- This is a time-based code and follows the 8-minute rule; a minimum of 8 minutes of direct contact is required to bill one unit.
- Manual therapy (97140) and Chiropractic Manipulative Treatment (98940-98943) may be billed on the same day only if performed in separate anatomical regions.
- When billing 97140 with CMT on the same day, modifier 59 (or X-modifiers) must be appended to 97140 to indicate a distinct procedural service.
- This code should not be reported if the provider is performing a simple massage for general relaxation, which is coded under 97124.
- Documentation must justify the medical necessity for skilled manual intervention over passive or self-stretching techniques.
- The 15-minute time increment applies to the total time spent across one or more regions.
