- Acute severe illness requiring hospitalization, such as severe pneumonia, acute myocardial infarction, or acute exacerbation of chronic obstructive pulmonary disease (COPD)

Procedure Steps
- Obtain a comprehensive history, including chief complaint, history of present illness (HPI), review of systems (ROS), and past, family, and social history (PFSH)
- Perform a comprehensive multi-system physical examination to assess all relevant body systems
- Engage in medical decision making of high complexity, which typically involves multiple diagnoses or management options, an extensive amount or complexity of data to be reviewed and analyzed, and a high risk of morbidity or mortality to the patient
- Order and interpret diagnostic tests (e.g., laboratory tests, imaging studies, electrocardiograms)
- Develop and document a comprehensive plan of care, including medication management, treatment modalities, consultations with specialists, and patient/family counseling
- Coordinate care with other healthcare professionals and services as appropriate
- Provide thorough documentation supporting all components of the E/M service and the rationale for the high complexity level
Coding Guidelines
- This code is used for the admitting physician's initial service to a patient for hospital inpatient or observation care on the date of admission.
- Documentation must clearly support the comprehensive nature of the history and examination, and the high complexity of the medical decision making.
- When billing based on time, 75 minutes of total time on the date of the encounter must be documented.
- Only one initial hospital inpatient or observation care code (99221-99223) can be reported by a physician for a patient during a single hospital stay.
- These codes should not be reported in conjunction with emergency department services (99281-99285) or office/other outpatient services (99202-99215, 99211-99215) on the same date by the same physician.
- Do not report initial hospital care codes for services provided in a skilled nursing facility, nursing facility, or other long-term care settings.
- For observation services that do not result in inpatient admission, these codes are used for the initial encounter if the patient meets the criteria for observation status and the service level.
- Subsequent hospital visits (99231-99233) are used for follow-up care provided after the initial visit.
