The transmission must be digital and automatic; patient-recorded data that is manually reported via telephone or a manual entry portal does not qualify for this code. The service is intended to monitor either acute or chronic conditions, allowing clinicians to observe trends and receive alerts if physiologic parameters fall outside of a predetermined range. This enables proactive management of the patient's condition between traditional face-to-face visits. This code does not include the time spent by the physician or clinical staff reviewing and analyzing the data, which are instead reported using codes 99457 and 99458.

Procedure Steps
- Ordering of the RPM service by a physician or qualified healthcare professional (QHP).
- Provision of an FDA-defined medical device to the patient for home use.
- Initial setup and patient education on device use (this specific step is billed under 99453).
- Automated recording and transmission of physiologic data from the device to the provider's monitoring platform.
- Verification that data transmission occurred for a minimum of 16 days out of the 30-day billing cycle.
- Maintenance of the monitoring platform to receive and store transmitted data.
- Notification of clinical staff or provider when programmed alerts are triggered by data outside of set thresholds.
Coding Guidelines
- Report 99454 only once per 30-day period, regardless of the number of devices provided to the patient.

- 99454 should not be reported if the monitoring period is less than 30 days.
- For new patients or patients not seen within one year, a face-to-face initiating visit (such as an E/M) is required before beginning RPM services.
- Do not report 99454 in conjunction with other remote monitoring codes like 95250 (CGM) or 94760 (pulse oximetry) if they are monitoring the same parameters.
- Clinical staff can provide services under the general supervision of the billing physician.
