A core requirement of this code is that it must include at least one instance of interactive communication with the patient or caregiver, such as a phone call or synchronous video session, to discuss the data and adjustments to the care plan. The monitoring involves physiologic parameters like blood pressure, heart rate, weight, or pulse oximetry. During the 20 minutes, the clinical staff or provider reviews the transmitted data, interprets the trends, assesses the patient's adherence to the current treatment, and modifies the care plan as necessary. Procedure Steps
- Collection and transmission of physiologic data from the patient-owned or provided device to the provider's platform.

- Analysis of the data in the context of the patient's existing medical history and current treatment goals.
- Conducting at least one interactive communication session (phone or video) with the patient or caregiver during the calendar month.
- Discussing the monitored results with the patient and providing education or instructions based on the findings.
- Adjustment of the clinical care plan, which may include changes in medication, diet, or activity levels.
- Documentation of the total time spent in minutes throughout the month and a summary of the interactive communication.
Coding Guidelines
- 99457 is reported once per calendar month, regardless of the number of physiological parameters being monitored.
- A minimum of 20 minutes of clinical staff, physician, or other qualified healthcare professional time must be spent to report this code.
- At least one interactive communication with the patient or caregiver is required to bill 99457.
- Do not report 99457 for time less than 20 minutes.
- 99457 can be reported in conjunction with 99458 for each additional 20 minutes of management time.
- Time spent on 99457 cannot be counted toward the time requirements for other services like Chronic Care Management (CCM) or Transitional Care Management (TCM) if those services are being billed concurrently.
- 99457 should not be reported for the same patient in the same month as 99091 (collection and interpretation of physiologic data).
- Devices used must meet the FDA definition of a medical device.
- The service is billed on a calendar month basis, not a rolling 30-day period.
