Generalized anxiety disorder

Generalized anxiety disorder (GAD) is a mental health condition characterized by persistent, excessive, and unrealistic worry about everyday things. This worry is often experienced as a lack of control and is associated with physical symptoms including muscle tension, restlessness, and sleep disturbances. Unlike other anxiety disorders, GAD is not focused on a specific object or situation but is pervasive across various domains of life such as health, work, and social interactions. To meet clinical criteria, the anxiety and worry must be present most days for at least six months.

Clinical Symptoms

  • Excessive anxiety and worry occurring more days than not for at least 6 months
  • Difficulty controlling the worry
  • Restlessness or feeling keyed up or on edge
  • Being easily fatigued
  • Difficulty concentrating or mind going blank
  • Irritability
  • Muscle tension
  • Sleep disturbance (difficulty falling or staying asleep, or restless, unsatisfying sleep)

Common Causes

  • Genetic predisposition (family history of anxiety disorders)
  • Brain chemistry imbalances involving neurotransmitters like serotonin, dopamine, and norepinephrine
  • Environmental stressors such as trauma, major life changes, or chronic work/relationship stress
  • Personality factors, such as a tendency toward neuroticism or behavioral inhibition
  • Chronic medical conditions that cause persistent physiological stress

Document the duration of symptoms to ensure they meet the 6-month threshold required for GAD.

Example: Patient reports daily feelings of uncontrollable worry regarding family health and finances for the past 8 months.

Specify the associated physical symptoms to distinguish GAD from other mental health conditions.

Example: Documentation includes presence of chronic muscle tension and significant sleep-onset insomnia associated with ruminative thoughts.

Explicitly state that the symptoms are not due to the direct physiological effects of a substance or a general medical condition.

Example: Thyroid function tests were normal, and symptoms do not correlate with substance use or medication side effects.
