How Do I Code Postpancreatectomy Diabetes Mellitus?
Quick answer
Postpancreatectomy diabetes mellitus takes three codes working together: a code from category E13 as the principal or first-listed diagnosis, E89.1 (postprocedural hypoinsulinemia), and a Z90.41- code for the acquired absence of pancreas — an explicit instruction in the Official Guidelines, not an inference.
Jump to the decision rule ↓Why it matters
The rule, verbatim, and why it is three codes
This page exists because coders search for exactly this phrase and the answer lives in a guideline subsection that is easy to miss. Section I.C.4.a addresses it by name:
“For postpancreatectomy diabetes mellitus (lack of insulin due to the surgical removal of all or part of the pancreas), assign code E89.1, Postprocedural hypoinsulinemia. Assign a code from category E13 as the principal or first-listed diagnosis and a code from subcategory Z90.41, Acquired absence of pancreas, as an additional code.”
The tabular says the same thing from every direction, which is how you can verify the chain yourself. E89.1 carries a Code First note — “if applicable, diabetes mellitus (postpancreatectomy) (postprocedural) (E13.-)” — plus Use Additional Code notes for the acquired absence of pancreas (Z90.41-) and insulin use (Z79.4). And Z90.41 reciprocates with its own Use Additional Code note for postpancreatectomy diabetes (E13.-) and insulin use. Three codes, each pointing at the others:
- E13.- — Other specified diabetes mellitus, with the fourth/fifth characters for any documented complications (principal / first-listed)
- E89.1 — Postprocedural hypoinsulinemia
- Z90.410 or Z90.411 — acquired total or partial absence of pancreas
Add Z79.4 when long-term insulin use is documented — category E13 carries the same control-agent Use Additional Code note as the other diabetes categories.
Why not E08, and why not E89.1 alone
The E08 trap. “Diabetes due to something” sounds like E08, Diabetes mellitus due to underlying condition — but E08’s own Excludes1 names “postpancreatectomy diabetes mellitus (E13.-)” and sends it away. E08 is for diabetes caused by a disease still present (Cushing’s syndrome, cystic fibrosis, pancreatitis); pancreatectomy is an event, and the classification routes its diabetes to E13.
The E89.1-alone trap. E89.1 by itself states the hypoinsulinemia but not the diabetes, drops the guideline’s explicitly first-listed E13 code, and loses the absence-of-pancreas status. Its own Code First note is the tabular telling you the code cannot stand alone when the diabetes is documented.
The documentation dependence is real: what the record must support is that the diabetes followed the pancreatectomy (total or partial). The E13 fourth and fifth characters then follow whatever complications are documented, exactly as in the other diabetes families — the combination-code logic is the same “with” convention that runs through E08-E13.
How to apply it
How to apply the rule
- Confirm the documentation: diabetes following surgical removal of all or part of the pancreas.
- Assign a code from E13 as principal or first-listed, with the complication character the record supports.
- Add E89.1, Postprocedural hypoinsulinemia.
- Add the Z90.41- code for the acquired absence of pancreas.
- Add the Z79 control-agent code(s); do not use E08 — its Excludes1 names postpancreatectomy diabetes.
Examples
Coding examples
- Diabetes after total pancreatectomy, no complications, on insulin. E13.9 → E89.1 → Z90.410 → Z79.4.
- Same, with diabetic CKD stage 3a. E13.22 → N18.31 → E89.1 → Z90.410 → Z79.4.
Common mistakes
Common mistakes
- E08 or E11 for postpancreatectomy diabetes.
- E89.1 alone without the E13 code.
- Omitting Z90.41-.
Documentation matters
Documentation that changes the coding
- The pancreatectomy and its extent (total or partial), which selects the Z90.41- code.
- The diabetes as a consequence of the surgery.
- Complications, coded with E13’s parallel characters.
Decision rule
Decision framework
| IF… | THEN… |
|---|---|
| Diabetes documented as following pancreatectomy | E13.- first, then E89.1, then Z90.41-, then Z79 |
| Diabetes due to pancreatitis or another pancreatic disease (not surgery) | The disease first, then E08.- |
| “Secondary diabetes,” cause not documented | E13.- (secondary diabetes NEC) |
FAQ
Which code is sequenced first for postpancreatectomy diabetes?
The E13.- code. The guideline states it directly: a code from category E13 is assigned as the principal or first-listed diagnosis, with E89.1 and the Z90.41- absence code as additional codes. The same order applies inpatient and outpatient — the guideline names both positions.
Is postpancreatectomy diabetes E08 or E13?
E13. Category E08 (diabetes due to underlying condition) excludes it by name — its Excludes1 reads “postpancreatectomy diabetes mellitus (E13.-)” — because E08 covers diabetes caused by a present underlying disease, while surgical removal of the pancreas is an event the classification routes to E13.
Do I still code E89.1 if the patient has diabetes codes on the claim?
Yes — the guideline assigns E89.1 alongside the E13 code, and E89.1’s Code First note sequences the diabetes ahead of it rather than replacing it. E89.1 carries the postprocedural hypoinsulinemia itself; E13 carries the diabetes and its complications.
Which Z codes belong on a postpancreatectomy diabetes claim?
Z90.410 (acquired total absence of pancreas) or Z90.411 (partial), per the guideline and per E89.1’s own Use Additional Code note — plus Z79.4 when long-term insulin use is documented, from the standard diabetes-category control-agent note.

Coder Takeaway: Three codes: E13.- first, E89.1 (postprocedural hypoinsulinemia), and Z90.41- (acquired absence of pancreas), plus the Z79 control agent — never E08, which excludes it.
